Medical pouches – are specialized organizer bags designed for carrying medications and essential consumables in field and combat conditions. The durability of materials and well-thought-out ergonomics literally determine the speed of providing aid and, consequently, the life of a soldier.

First aid pouch (medical): types and operational features
The key characteristics of a tactical medical pouch come down to three points: dimensions, carrying method, and internal organization. Their combination defines the actual volume of the IFAK and how much time passes from the moment you reach for the kit to the start of assistance. Four main types of this gear can be distinguished.
Horizontal medical pouches
These models are built with a two-component design: a base rigidly mounted on the gear and a removable organizer-first aid kit fixed in the base. This solution allows you to pull out the pouch in a fraction of a second and start working with the contents, regardless of where it is mounted on the equipment. General features include:
- orientation – horizontal, opening in a “book” format or with a flip-down “lid”;
- linear layout. Items are arranged in a single row for instant overview and quick retrieval;
- intended for compact IFAK sets – from minimal to extended kits.
Medical pouches of this format mainly differ in size. The most compact – a low-profile, miniature option with relatively spacious inner storage. The main advantage – does not interfere when sitting in a vehicle or working seated, does not conflict with other gear on the belt/plate carrier. Optimal for a basic IFAK with priority on minimum bulk. Medium-sized horizontal medical pouches – a more capacious and convenient organizer for a more distributed layout of medical supplies. However, dimensions are no longer as “flat,” though in most use scenarios, no critical discomfort arises. A good choice for those who frequently move in vehicles. Large horizontal medical pouches – designed for extended kits, suitable for cases where capacity and detailed segmentation matter more than compact gear profile.
Vertical first aid pouch
Differs from horizontal ones with several practical features:
- Vertical layout with top access (most often) – convenient for retrieving contents on the go.
- Additional straps and loops for attachment to tactical vests, backpacks, and other gear.
- More advanced internal organization – easier to segment the kit and quickly find needed items.
- Some models feature a carry handle – easy to carry by hand and quickly attach to gear when required.
TQ (tourniquet) pouches
A medical pouch for a tourniquet can be purchased with two different mounting types. MOLLE interface: a modular system allowing rigid attachment to vests, platforms, belts, and any MOLLE-compatible gear. Secure fit and compatibility with most wearable systems. VELCRO fastener: fixation on the velcro panel of a plate carrier or other gear. Tourniquet pouches of this type can support both right- and left-handed extraction, simplifying use depending on the dominant hand and placement scheme.
The most significant difference between models lies in access to the contents.
Open tourniquet pouches
The design without a flap ensures instant access: part of the tourniquet protrudes outward, making it easy to grab and deploy immediately. Despite being open, retention is designed for active movements – the tourniquet will not fall out even during very intense maneuvers. Drawbacks include: the exposed part is constantly in contact with dust and dirt, and snagging on branches/obstacles poses a real risk of loss.
Semi-closed medical tourniquet pouches
Partial protection and fixation are provided by:
- a flap strap – in MOLLE versions;
- side elastic bands – in velcro-mounted versions.
Semi-closed modifications do not slow access to the tourniquet, significantly reduce the risk of accidental loss, but the sides remain exposed, so protection from the environment is limited.
Closed first aid pouch for a tourniquet
The tourniquet is fully covered with a flap – maximum protection from dust, moisture, and accidental snags. Access is simple: pull the strap, the flap opens, and the tourniquet is ready for use. Advanced models may feature additional useful elements:
- Medical marking (cross) on the front – quick identification among other gear.
- Additional velcro panel – placement of patches, blood type identifiers, unit tags.
- Opening limiter – when closed, prevents contents from falling out, while access to the tourniquet is only from the top of the pouch.
In addition to all the variations above, there are also “pocket-sized” medical pouches. These are mini-organizers for a basic first aid kit: tablets, band-aids, ampoules, and other small items. When folded, they are comparable in size to a small wallet, making them easy to place anywhere – in a pocket, backpack, or inside a horizontal or vertical medical pouch.
